A Good Idea

Description

The Complex Care Program (CCP), founded in 2002, provides a medical home for children and youth with special healthcare needs. CCP focuses on family-centered, coordinated, compassionate, comprehensive, and continuous care. Integrated within the Children's Health Center at Children's National Medical Center (CNMC), CCP provides both primary care and consultative services to children with two or more complex chronic conditions. The program is partially funded by the State of Maryland Department of Health and Mental Hygiene-Office of Genetics and Children with Special Health Care Needs, the DC Department of Health-Community Health Administration, and Children's National Medical Center-Goldberg Center for Community Pediatric Health.

Goal / Mission

CCP aims to provide ongoing comprehensive care coordination to children with medically complex, chronic conditions within Children's National health care system.

Results / Accomplishments

The program has successfully provided comprehensive care coordination through a team including a nurse case manager, parent navigators, and a social worker to over 140 patients during fiscal year 2012, from July 1, 2011 to June 30, 2012.
